ANNOUNCEMENT.

The Department of Veteran Affairs, VA Dayton VA Medical Center (VAMC), 552, hereafter referred to as VA, located at 4100 W. Third Street; Dayton, Ohio 45428 requires the Contractor to provide services to VA eligible homeless Veterans through a Contracted Emergency Residential Services (CERS) model of residential care as part of the Healthcare for Homeless Veterans (HCHV) Program. These services, not to exceed 20 Veterans, at any one time, are to be provided to homeless Veterans, both chronically and non-chronically homeless as defined in the Statement of Work (SOW).Â

The Department of Veterans Affairs, Dayton VAMC intends to enter into a MULTIPLE Award Indefinite Quantity, Indefinite Delivery Contract (IDIQ), based on the contents of this solicitation. It is anticipated that the length of stay for homeless Veterans admitted to the CERS will be up to 90 days with limited exceptions for stays greater than 180 days. Geographic region to be Montgomery County, Ohio and Wayne County, Indiana. Beds for the Montgomery County, Ohio geographic region will not exceed 15 beds per day, and the Wayne County, Indiana geographic region will not exceed 5 beds per day.

The goal of the HCHV program is to provide housing and supportive case management services to eligible Veterans with complex needs and facilitate their access to a broad range of medical, mental health, and rehabilitative services utilizing HCHV Contracted Emergency Residential Services guidelines, hereafter referred to as CERS. Â The Dayton VAMC HCHV Program is a Housing First Model.

HCHV CERS programs target and prioritize Veterans transitioning from literal street homelessness, Veterans temporarily residing in local community shelters, Veterans being discharged from institutions who are homeless, homeless Veterans with co-occurring mental health or substance use disorders, and Veterans who recently became homeless and require safe and stable living arrangements while seeking permanent housing or treatment options. The facility will not be used for detoxification or other hospital level treatment. Dayton VAMC will serve as the treatment center.

The Dayton VAMC HCHV Program prohibits discrimination based on age, sex, race, color, language, gender, sexual expression, gender identity and expression, socioeconomic status, as well as homeless Veterans with medical and mental disabilities that allow them to participate in housing services safely and clinically at the CERS level. The contractor will serve both individual male, female, and transgender Veterans. In addition, the contractor will serve VA eligible homeless Veterans with a legal status of Sex Offender (SO).

Eligibility will be determined by the VA and is based on Veteran status. The Veteran must meet the McKinney-Vento Homeless Assistance Act definition of homelessness, (see http://www.hudhre.info/hearth/ for additional information), and are determined to have a serious medical, mental health, substance abuse or other serious psychosocial conditions or stressors making the Veteran highly vulnerable with a high likelihood of death due to homelessness.Â

This is an indefinite delivery/indefinite quantity contract for the period, 1 February 2024 to 31 January 2029 in accordance with FAR 16.5.Â

Requested submittal Information:
Contractors having the skills and capabilities necessary to perform the stated requirements should submit a response of no more than 10 pages in length, single spaced, 12-point font minimum. The Government will not review any other data or attachments that are in excess of the 10 pages. Companies should provide clear and unambiguous evidence to substantiate their capability to fulfill these requirements.

15. Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities/qualifications/skills your company possesses to perform services described in the scope of work.

16. Do you need to subcontract any portion of work to provide these services stated herein?

17. Past Experience: Brief summary of the company history relative to similar requirements. Offer may provide the following information on a maximum of three similar projects for which the responder was a prime or subcontractor.

a. The name, address, and value of each project.
b. The Prime Contract Type, Firm Fixed-Price, or Time and Material.
c. The name, telephone and address of the owner of each project.
d. A description of each project and why it is relevant to this requirement. Include difficulties and successes.
e. Your company s role and services provided for each project.

18. What are the common qualifications of the people who are providing these services?
