Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with (IAW) the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Government reserves the right to make no award from this solicitation. Â
This is a request for quote (RFQ), and the solicitation number is 36C25023Q0066. The Government anticipates awarding a firm-fixed price Purchase Order resulting from this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circulars 2022-08, effective 09/23/2022. The associated NAICS Code is 334510, Electromedical and Electrotherapeutic Apparatus Manufacturing. Â
The Department of Veterans Affairs, Network Contracting Office 10, is soliciting quotes from all sources to supply the Chalmers P. Wylie Veterans Affairs Ambulatory Care Center Columbus, Olympus Flexible scopes for ENT clinic procedures. Respondents to this solicitation must fully demonstrate their capability by supplying detailed quote information, along with any other documents necessary to support the requirements below. However, we believe only Olympus and Olympus Authorized Distributor are the only Contractors able to provide this requirement.
Quotes are to be provided to Gina P. Crank, Contracting Officer, via email at gina.crank@va.gov, no later than October 21,  2022, by 6:00 PM EST. No telephone calls will be accepted. No submission received after the above-mentioned date and time will be accepted.Â

STATEMENT OF WORK (SOW)
1. Background. The Chalmers P. Wylie VA is in need of more Flexible scopes for ENT clinic procedures. We currently have 3 on hand but with the increased need and desires of the veterans we care for we are in need to increase our par level of how many scopes we have on hand. We are starting to see more veterans and they all have expressed that they like the smaller flexible scope to the slightly larger rigid scopes. We have also recently have had to send out a few of the current scopes we have on hand for repair, thus leaving our scope numbers low for patient care. The turnaround from SPS from patient to patient is over 24 hrs. d/t the required dry time. So, more scopes are urgently needed for our facility to continue with patient care.
2. Scope. We are looking to purchase 6 more of the Flexible slim Rhino laryngoscope with pistol grip the ENF-V4. In addition to the 6 scopes, we need to purchase the proper trays and mats required for reprocessing and sterilization is also required. These items are needed to accommodate the need of the veterans in the ENT clinic setting to receive the best care and required testing to properly evaluate the needs of the veterans.
3. Specific Tasks. As the contractor deems necessary for Acquisition of surgical scope.

4. Performance Monitoring

Scope and associated items will be inspected by HTM and SPD departments upon arrival to facility. Proper protocol as per outlined by scope specifications for use and sterilization will be followed.

Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government.
The award will be made to the response most advantageous to the Government.
Responses should contain your best terms and conditions.
To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:

Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The equipment proposed for purchase is the ENF-V4 Flexible Slim Rhino Laryngoscope with pistol grip. This scope is intended to be used in the ENT clinic for in office procedures. This is a flexible and small in diameter scope that is more comfortable for the veterans when they are in the ENT clinic for evaluation of Sinus issues.

Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Based on the existing Olympus Tower, the requested requirements are only compatible with the Olympus system already in place in the ENT clinic for scope usage.

Description of market research conducted and results or statement why it was not conducted: Market Research was conducted by Internet, GSA, NAC and VIP. VIP search with NAICS 334510 resulted 184 SD/VOSBs, added keywords resulted in 5 SD/VOSBs. However, Olympus and their exclusive Authorized SDVOSB Distributor, Alliant Healthcare Products, are the only distributors of the Olympus items. The VA Rule of two is Unattainable.
