Amendment 2: 17 November 2022 Site Visit Questions and Answers



ATTACHMENT 5 has been publised with the complete list of questions and answers from the 17 November 2022 site visit.





Amendment 1: Site Visit



A site visit has been scheduled at the Wright Patterson Medical Center on November 17th, 2022, 9 a.m. EST, in the TOPA conference room. Attendees will meet in the entry of the Medical Center. Please RSVP with your list of attendees to tracy.ferrise@us.af.mil and kaitlyn.wolfe@us.af.mil, by November 15th at 4:00 p.m. EST. Should any members of the team require base access, include the following information in the RSVP email:



* Full name exactly as it appears on their driver's license/identification card (no nicknames or go-by names)



* Visitors e-mail address



* Visitors full driver's license/identification card number and the state of issue



* Ensure they are a U.S. Citizen



The Contracting Office plans to record a list of the questions and answers asked during the site visit. The questions and answers will be posted to the SAM solicitation for situational awareness.





COMBINED SYNOPSIS/SOLICITATION



This is a combined synopsis/solicitation, which is expected to result in a multiple award of a five (5) year Blanket Purchase Agreement (BPA) for commercial service, Ambulance Services, as described under the paragraph below titled “Statement of Work.” The solicitation is prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This will be a competitive FAR Part 13 acquisition using simplified acquisition procedures and FAR subpart 12, Acquisition of Commercial Items. This acquisition is not subject to FAR 15.3 source selection procedures. Accordingly, the Government is not obligated to determine a competitive range, conduct discussions with all or any Offerors, solicit final quote revisions, or use other techniques associated with FAR 15.3.



This solicitation incorporates FAR provisions and clauses in effect through Federal Acquisition Circular (FAC) FAC 2020-04, effective 1 January 2020; Defense Federal Acquisition Regulation Supplement (DFARS) Publication Notice (DPN) 20200114, effective 14 January 2020; and Air Force Federal Acquisition Regulation Supplement (AFFARS) Air Force Acquisition Circular (AFAC) 2019-1001, effective 1 October 2020.



It is anticipated that approximately two (2) BPAs will be awarded from this solicitation based upon the two (2) lowest priced technically acceptable (LPTA). Quantities listed as estimates are based on historical consumption and are not to be construed as purchases that the Government will actually make under the BPA(s). The Government will only be obligated to the extent of calls placed against an awarded BPA(s). Once the BPA(s) are awarded the Government will rotate vendors under their discretion and vendor availability. An award(s), if any, will be made to the responsible Vendor(s) who submits a quote that:




  1. Conforms to the requirements of the solicitation;

  2. Receives a rating of “Meets” on Technical Capability by meeting the requirements contained in the Statement of Work;

  3. Submits the quote with the lowest total evaluated price (TEP), provided that the TEP is not unbalanced and is determined to be fair and reasonable; and

  4. The offeror must be determined to be responsible based on the standards in FAR 9.104.

The quotations may be in any format but MUST include:



1. Proposing company’s name, address, SAM Unique Entity ID, Cage Code, and TIN



2. Point of contact’s name, phone, and email



3. Price List number & date



4. Timeframe that the price list is valid – normally a minimum of one year



5. Completed copy of Offeror Representations and Certification (attached)



If the offeror has not completed the representations and certifications listed above online in the System for Award Management (http://www.sam.gov), the offeror shall complete these representations and certifications (as prescribed therein) and submit them with the offeror’s price quote.



Offerors shall complete the price list spreadsheet attached to the solicitation and return as part of the quote. Quotes shall consist of two separate parts, a technical quote, and a price quote.



Acceptable means of Submission: All submissions must be submitted electronically to the following email addresses, tracy.ferrise@us.af.mil and kaitlyn.wolfe@us.af.mil, by November 21st by 4:00 p.m. EST.



Any correspondence sent via e-mail must contain the subject line “Solicitation FA8601-23-Q-0019”. The entire quote must be contained in a single e-mail that does not exceed 5 megabytes including attachments, if any. E-mails with compressed files are not permitted. Note that e-mail filters at Wright-Patterson Air Force Base are designed to filter e-mails without subject lines or with suspicious subject lines or contents (i.e., .exe or .zip files). Therefore, if the specified subject line is not included, the e-mail may not get through the e-mail filters. Also be advised that .zip or .exe files are not allowable attachments and may be deleted by the email filters at Wright-Patterson. If sending attachments with email, ensure only .pdf, .doc, .docx, .xls or .xlsx documents are sent. The email filter may delete any other form of attachments.



A confirmation email will be sent once the quote is received. If you do not receive a confirmation e-mail within 24 hours of submitting the quote, please call or e-mail the point of contact listed on the solicitation. Address questions regarding this solicitation to Tracy Ferrise via e-mail at tracy.ferrise@us.af.mil.



To be considered for award, a vendor must meet all minimum required items in the Statement of Work (attached). The Government intends to evaluate the quotes and award a contract without interchanges with vendors. Therefore, the vendor’s initial quote should contain the vendor’s best terms from a price and technical standpoint. The Government reserves the right to conduct interchanges with one, some, none, or all vendors, if the evaluation team later determines them to be necessary. Submittal of quotes in response to this combined synopsis/solicitation constitutes agreement by the Offeror to all terms & conditions contained herein, which will also be the terms & conditions of any resulting contract. Failure to agree to the terms & conditions may result in the quote to be determined non-responsive and as a result may not be selected for award. It is the Offerors responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions in full text may be accessed via the Internet website https://www.acquisition.gov.
