Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with (IAW) the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Government reserves the right to make no award from this solicitation. Â
This is a request for quote (RFQ), and the solicitation number is 36C25023Q0398. The Government anticipates awarding a firm-fixed price Purchase Order resulting from this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circulars 2022-06, effective 05/01/2022. The associated NAICS Code is 339113, Surgical Appliance and Supplies Manufacturing. Â
The Department of Veterans Affairs, Network Contracting Office 10, is soliciting quotes from all sources to supply the Columbus VA Medical Center with Exam Tables, must fully demonstrate their capability by supplying detailed quote information, along with any other documents necessary to support the requirements below.

2. Contract Title. Daniel L. Kinnard CBOC Clinical Expansion Pro-glide 300+ and Urgent Care Center.
3. Background.
The Chalmers P. Wylie VA needs 7 new Pro-Glide 300+ patient exam tables. These exam tables will be used for the new Daniel L. Kinnard CBOC clinic expansion and Urgent Care Center.
4. Scope.
Pro-Glide 300+ patient exam tables will be delivered to Chalmers P. Wylie VA; this will allow for the continued standardization of the beds within the facility for Bio-Med and Logistics to stock supplies and service. This also allows for the VA to meet the infectious disease requirements for cleaning procedures for patient safety.

No maintenance or installation of a VA Information System Hardware or Software is performed under this contract. No contractor access to a VA Information System, VA Protected Health Information (PHI), nor VA Personally Identifiable Information (PII) is allowed or performed under this contract. Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security.

12. Period of Performance.
Delivery schedule will be determined and agreed upon with Contractor at the time of award.

All quoters shall submit the following: A completed schedule with pricing, adequate documentation detailing the capabilities of the product.

Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government.
The award will be made to the response most advantageous to the Government.
Responses should contain your best terms and conditions.
To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:
"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."
